2. Official high school transcript and/or official GED transcript
3. Official college transcripts, if applicable
4. A writing sample or personal essay
4. Letter(s) of recommendation from an employer, teacher, or guidance counselor
5. Optional proof of volunteer hours for the fields of Physical Therapy Assisting
6. Test of English as a Foreign Language (TOEFL) scores for all students for whom English is a second language
